Ph.D. Application Checklist

____ Completed and submitted electronic application (preferred) or completed and mailed paper application form.

____ Paid application fee.

____ Graduate Record Examination (GRE) scores sent to department.

____ Test of English as a Foreign Language (TOEFL) scores sent to department if native language is not English.

____ Official transcripts from all institutions, in sealed envelopes, sent to institution.

____ Three (3) letters of recommendation submitted via the online application (preferred) or submitted in sealed envelopes to the department.

____ Statement of puspose (approximately two pages) completed as part of the online application or sent to the department.

____ Departmental application. This is automatically part of the online application, referred to as "Departmental Questions." If you do not apply online, you may download a copy of the departmental application and print it for inclusion with a paper application.

____ Complete items above.

____ Create a Cognitive Science or Program in Neuroscience online application and save it. Do not submit it.

____ E-mail us to state that you have completed all of the above steps and are ready to apply to the second program. Allow two (2) days for processing.

____ After two days, return to the second application (Cognitive Science or Neuroscience) and submit it. You will not be charged an additional application fee.
